Call Our Office to Book An Appointment
We're accepting new patients! So whether it's for a sick visit, a regular check-up, or to get your camp forms completed, call our office to set up an appointment at either location:

Download the Patient Registration Form Packet, fill it out, and email it to us prior to your visit.

Come and visit our colorful, fun and warm space designed with kids in mind. Get seen by our loving and caring team of doctors who are passionate about providing exceptional care for your family.
What to bring:
- Insurance cards and photo ID
- Completed registration forms (if not already emailed)
- Your child's immunization records
- List of current medications or vitamins
- Any previous medical records from your child's former pediatrician
Plan to arrive:
- 15 minutes early for your first visit to complete any remaining paperwork
- With extra time for parking and finding our office
Switching practices? Download this Authorization For Release Form and email it completed to our office so we can retrieve your records.

We offer two ways to get to know us before your little one arrives.
1:1 Private Prenatal Visit
West 60th Street Office
A free & private, one-on-one visit with one of our providers. Tour the office, ask all your questions, and get to know us before baby arrives. Available Monday–Friday, 11am–3pm.
Book a VisitWeekly Zoom Session
Free, no commitment
Join an informal group session via Zoom to meet the practice, ask questions, and get a feel for who we are, all from home. Every Tuesday at 7:30pm via Zoom
